When creating forms, collecting a person's name can be done flexibly using different field configurations. Here's how to set up name fields effectively:
When to Use the Name Field
You can either:
- Use the Name Field for a streamlined name input experience.
Use Short Text Fields for more control, such as:
- Asking for the last name first.
- Splitting name components across two rows.
Tip: To customize with Short Answer Fields, simply add two fields, change the labels, and choose "2 Columns" under Form Extras.
How to Add a Name Field
To add a Name Field:
Go to the Basic inputs section on the left-hand side of the form builder.
Click and drag the Name Field into your form from left to right.
Customize your Field
After selecting the name field, you can customize it on the left-hand side under the general tab.
Optional Sub-Fields
The default Name Field collects First Name and Last Name, but you can optionally include:
- Prefix (e.g., Mr., Ms., Dr.)
- Middle Name
- Middle Initial
- Suffix (e.g., Jr., III)
To enable these, check the corresponding boxes under the field-specific settings on the left-hand side.
Note: If the Name Field is marked as required, all selected sub-fields will also become required. For example, if you require first and last names and include a middle name, the middle name field will also be mandatory.
