Note: This article applies to the new Form Step experience in Streamline.
Name fields are used to collect a respondent’s full name in a structured format.
They’re located under the Contact Information category in the form builder and are ideal for capturing individual name details such as first, last, and additional components like prefix or suffix.
Adding a Name Field
- In the form editor, click Add Field.
- From the Contact Information category, select Name.
- The field will appear on your form, ready for configuration.
Customizing Field Properties
After adding the field, you can customize how the Name field appears and behaves directly within the form builder.
You can:
- Change the field title to match your form’s tone or requirement (for example, “Full Name” or “Contact Name”).
To the right of the field title, you’ll also find quick-access icons to manage visibility and behaviour:
- Toggle Required on or off.
- Click the eye icon to hide or show the field.
- Click the lock icon to make the field read-only.
- Click the two boxes to duplicate the field.
- Click the trash can to delete it.
Field Settings
In the field settings panel on the top right, you’ll find options to enable or disable specific components of the Name field:
- Data Mapping Label: Add an internal label for mapping form data to integrations or styling with CSS.
- Prefix: Enable a prefix dropdown (for example, Mr., Ms., Dr.) to capture titles or honorifics.
- Middle Name: Allow users to include a middle name or initial.
- Suffix: Include a suffix field (for example, Jr., Sr., III) for additional name details.
These options give you flexibility to decide how detailed the name capture should be for your specific use case.
Tips for Using Name Fields
- Enable only the fields you need to keep the form streamlined. For example, omit prefix and suffix unless they’re relevant to your workflow.
- Use a clear field title such as “Full Name” to help respondents understand what’s expected.
- For forms that require company or organizational information, pair the Name field with a Short Answer field for “Company Name.”
