Note: This article applies to the new Form Step experience in Streamline.
Phone fields are used to collect valid phone numbers from respondents.
They’re located under the Contact Information category in the form builder and are useful for gathering contact details for communication, verification, or follow-up.
Adding a Phone Field
- In the form editor, click Add Field.
- From the Contact Information category, select Phone.
- The field will appear on your form, ready for configuration.
Customizing Field Properties
After adding the field, you can customize it directly within the form builder.
You can:
- Change the field title to specify the type of number being collected (for example, “Mobile Number” or “Business Phone”).
- Add helper text below the field to provide formatting guidance or context (for example, “Include country code” or “We’ll only use this for account updates”).
- Add placeholder text to show an example format, such as “(123) 456-7890.”
To the right of the field title, you’ll also find quick-access icons to manage visibility and behavior:
- Toggle Required on or off.
- Click the eye icon to hide or show the field.
- Click the lock icon to make the field read-only.
- Click the two boxes to duplicate the field.
- Click the trash can to delete it.
Field Settings
In the field settings panel on the top right, you’ll find options to configure how the Phone field behaves:
- Data Mapping Label: Add an internal label for mapping form data to integrations or for use with CSS styling.
- Default Value: Pre-fill the field with a default number, such as a company contact line or example phone format.
These settings allow you to ensure consistent data mapping and streamline form completion.
Tips for Using Phone Fields
- Use phone fields to capture verified contact information for users or customers.
- Provide clear helper text if a specific format is required (for example, “Enter numbers only” or “Include area code”).
- Use the default value setting for prefilled examples or internal workflows.
- Combine with Email or Name fields to create a complete contact information section in your form.
